*Job Overview::*
Join our Primary Care Team and coordinate the timely care for patients!
You will work with a variety of patient populations and those with chronic disease(s) who receive care in clinical and home-based setting. Track and maintain case management/coordination to ensure timely referrals and scheduling. Monitor patient progress and track outcomes.

*Education:*

* Associate’s degree or 2 years of college preferably in social work, social services, healthcare administration, business administration, or related field *OR *2 years of relevant professional experience may be exchanged for a degree or course work.

*Experience Required*

* 2 years of healthcare, behavioral health, dental, or relevant administrative experience required.
* A Bachelor’s degree in a relevant field may be exchanged for all required experience.
* Basic Life Support preferred.

Pay: $26.06 - $36.08 per hour
